Important: If you have had multiple failed login attempts, and were unable to access your UR Medicine MyChart account, we have deactivated your account for security reasons; you'll receive an email when you account has been deactivated. To reactivate your account, please contact the MyChart Customer Service Center 8 a.m. to 4:30 p.m. weekdays: submit an online form. After verifying your identity, we will give you a temporary password to use to login; you'll then be asked to create a new password.
