Access for My Kids/Family/Friends

Create a UR Medicine MyChart Account in 3 Easy Steps:

  1. Complete an online form
  2. Receive an activation code
  3. Activate your account

What Type of Access Do You Need?

Only parents or legal guardians may request access to their child's account. Depending on your child's age, you will be given either full access (birth to 11 years of age) or limited access (12 to 17 years of age). See "Proxy Access for Children" below for more details.

To access your child's account, you'll first need your own UR Medicine MyChart account; choose the correct selection below to get started:

You can request access to your child's MyChart from within your MyChart account. Click here to login to your MyChart account; then fill out the form found by clicking on Settings, then Family & Friends. We will review your request, and send a reply to your MyChart account (within the week up to 14 business days).

Please fill out the online Activation Code Proxy Request form. We will review your request, and send a MyChart activation code via email (within the week up to 14 business days). Once you activate your MyChart account, you can then view your child's account by clicking on the tab with their name (on the right side of screen).

Additional Information

  • Birth to 11 years of age: Parents and legal guardians are provided with full access to their child's MyChart. A week before your child's 12th birthday, you will receive an email alerting you that your access will be transitioned to a limited proxy access (see below) the day before your child's birthday.
  • 12 to 17 years of age: Parents and legal guardians are provided with limited access to their child's MyChart (able to view your child's allergies, immunizations, and letters). Receiving full access to your child's chart begins once your child has an appointment and receives a MyChart account. A MyChart message is then sent to your child asking him/her to approve proxy access (only available to parents or legal guardians). The child can choose to provide Full Access (recommended) or Full Access (Without Visit Notes)**. If you already have full access to your child’s account, a week before his/her 12th birthday, you will receive an email alerting you that your access will be switched to Limited Access off the day before your child's birthday.
  • Diminished Capacity Children: Parents and legal guardians of a child with diminished capacity are provided with full access to their child's MyChart until the child's 18th birthday. Authorized representatives of patients with diminished capacity who are over 18 years of age may request proxy access through their child's doctor's office.
  • **Full Access (Without Visit Notes): This means your proxy will be unable to view any Visit Notes. UR Medicine is providing access to many visit notes made by your providers. By referring to these notes, you can gain a better understanding of your health, take more active steps to improve your health and build a closer relationship with your care team. (I want to view another adult's MyChart, or I want to allow others to view my MyChart)

Proxy access can only be granted when an adult (18 years or older) gives permission for other adults to have access to all information in their MyChart.

To grant permission for another adult to access your MyChart as a proxy, choose the correct selection below to get started:

Great News! You can now grant permission for proxy Full Access (recommended) or Full Access (Without Visit Notes)** for another adult from within your own MyChart account (with the exclusion of billing information). Click here to login to your MyChart account; then fill out the form found by clicking on Settings, then Family & Friends. We will review your request, and send a reply to your MyChart account (within the week up to 14 business days).

If you are 18 years or older, you can fill out the Online Access Request Form to activate your MyChart account. Then, simply follow the steps in the "I have a MyChart Account" selection above.

What this means: I do not want a MyChart Account, but I want to give permission to another adult to have access.

Please click here to download and fill out the MyChart Proxy Authorization: 18 and Over form. You will need to give this form to your doctor's office, mail the form to the address provided on the form, or fax the form to the fax number provided on the form. Please allow at least two weeks for processing; an activation letter will be mailed to your proxy.

Additional Information

  • Diminished Capacity Adults: Authorized representatives may request proxy access through a patient's UR Medicine doctor's office. Access cannot be requested online at this time.
