MYCHART TERMS AND CONDITIONS OF USE AND AUTHORIZATION FOR RELEASE OF INFORMATION AGREEMENT

Strong Memorial Hospital, Highland Hospital, and FF Thompson Hospital, their doctors and clinics, and other participating physicians (hereafter referred to individually for convenience as "Hospital") are pleased to offer you access to your medical information via a Web-based version of our computer systems called MyChart. To request an account, read, and complete this form, indicating you have read, understand and agree to these Terms & Conditions of Use and that you authorize the release of information via MyChart. These Terms & Conditions outline our practices and our sensitivity to your right to privacy. Hospital does not guarantee that you will be able to access MyChart, its availability, or content completeness or accuracy, and reserves the right to revoke MyChart access at any time for any reason.

Authorization for Release of Information Via MyChart
By accessing or using Hospital's MyChart, I agree to be bound by the terms and the conditions of this Agreement and Authorization and request that medical information regarding your care and treatment at Hospital be released to you and through online access to MyChart. This authorization will remain valid unless/until it is revoked.

I understand that this authorization may cover disclosure of information relating to ALCOHOL or DRUG ABUSE, GENETIC TESTING, PSYCHIATRIC CARE and/or CONFIDENTIAL HIV RELATED INFORMATION. In the event the medical information described below includes any of these types of information, I specifically authorize release of such information via MyChart. I also understand that I have the right to cancel this release of information through MyChart at any time.

I understand that I have a right to request a list of people who may receive or use my HIV related information without authorization. If I experience discrimination because of the release or disclosure of HIV related information, I may contact the New York State Division of Human Rights at (212) 480-2493 which i.e. responsible for protecting my rights.

Response to Electronic Communication
Emergency situations requiring immediate attention should not be submitted electronically. Hospital will try to provide response to electronic inquiries. However, delays may occur based upon message volume, availability of staff, the complexity of the patient's condition, or technology disruptions. Therefore, Hospital cannot guarantee that messages will be responded to, so you should allow at least 72 hours for a response. We are only able to respond to patient communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide reliable or accurate response.

Web Site Links
MyChart may offer links to related medical Web sites not managed by Hospital. These Web site link(s) are for a patient's informational purposes only. Hospital does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.

E-mail Privacy
Patients who are users of MyChart should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to a patient's e-mail will be able to see this notification. This could include members of your family or anyone else that can access to your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address or using MyChart. If you send us an e-mail communication, it may be shared with the Hospital staff that assists the physician in providing the patient's medical care. A patient's confidential medical information on MyChart will be accessible only to appropriate clinical staff.

Security
User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a Hospital MyChart user, your role in maintaining the security of your medical information is to change your password on a regular basis and to keep your MyChart ID name and password confidential. This is part of your MyChart Terms and Conditions of Use and Authorization contained in this document.

This Website uses encryption software, and Hospital has implemented reasonable and appropriate security measures to protect your medical information. When using Hospital's MyChart, information will be transmitted over an Internet medium that is beyond the control of Hospital and its contractors. You are responsible for ensuring that the computer system(s) you use to access MyChart have the appropriate security controls enabled.

Mobile Device Platform
In addition to viewing MyChart from your computer or laptop, you can also access MyChart via a mobile device such as an iPhone or Droid. The mobile application has most, but not ALL, of the MyChart functionality. You will still be able to view test results, medications, view/send messages, but you will not be able to request a medication refill or request other changes to your chart. If you elect to view MyChart via a mobile device, your use via a mobile devise or any other means is subject to the Terms and Conditions of this Agreement and Authorization.

Medical Record Data

  1. As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including results of all test, procedures and therapies. NOT ALL of your Hospital medical record may appear on your MyChart. Posting of information is at the discretion of your individual health care providers(s) and Hospital.
  2. Your health care provider(s) will maintain and update your electronic record at each office visit. However, if you believe any information displayed in MyChart is not accurate, you are encouraged to notify your health care provider at your next office visit or submit a customer service request.
  3. Hospital reserves the right in the future to charge a fee for the use of MyChart. You will be notified of this change and asked to re-apply if you agree to the fee and wish to continue with this service.

Prohibited Actions
You agree that you will not upload or transmit any communications or content of any type (including secure messaging) that may infringe upon, misappropriate or violate any rights of any party and that your use of MyChart will not include any of the following:

  • signing on as or pretending to be another person
  • transmitting messages or material that infringes on or violates the intellectual property rights of others or the privacy or publicity rights of others
  • transmitting material or content in any form or media, including email or other electronic communication that is unlawful, obscene, defamatory, predatory of minors, threatening, harassing, abusive, slanderous, or hateful to any person (including Hospital personnel) or entity
  • using MyChart's interactive services in any way that is intended or is reasonably likely to harm the patient or others
  • collecting information about others, including e-mail addresses
  • intentionally distributing viruses or other harmful computer code

Information Does Not Constitute Medical Advice
Hospital assumes no responsibility for how you use the information you obtain from this Website. You should consult with a physician or other health care provider regarding your own condition and how this Website content may or may not apply to you. You may be accessing your own medical information via the MyChart application and may wish to communicate with one of your health care providers about that or related information. Neither MyChart nor any other application available through this Website is a substitute for appropriate and timely contact with your health care provider. This Website is not a substitute for consultation with your health care provider. You should never change or stop any course of treatment prescribed by your health care provider without first consulting him or her.

Accuracy and Integrity of Information
Although Hospital attempts to ensure the integrity of the Website, it makes no guarantees whatsoever as to the correctness or accuracy of the Website. It is possible that the Site could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy arises, please inform Hospital so that it can be corrected. Information contained on this Website may be changed or updated without notice.

PROXY MYCHART ACCESS
If you are accessing someone else's MyChart information (proxy access), you agree to be bound by the terms and conditions above and agree to comply with the following requirement: There is the possibility that the information disclosed to you via MyChart may include confidential HIV information protected by state law. State law prohibits you from making any further disclosure of HIV information without the specific written consent of the person to whom it pertains, or as otherwise permitted by law. Any unauthorized further disclosure in violation of state law may result in a fine or jail sentence or both. A general authorization for the disclosure of medical or other information is NOT sufficient authorization for further disclosure.

UR Medicine MyChart Alerts

Program Description
UR Medicine MyChart Alerts allows mobile subscribers to opt-in to receive information from UR Medicine (Strong Memorial Hospital, Highland Hospital, and FF Thompson Hospital, their doctors and clinics, and other participating physicians) through SMS messages straight to the subscriber's phone.

Supported Carriers
UR Medicine MyChart Alerts is available on the following carriers: Alltel, Appalachian Wireless, AT&T, Bluegrass Cellular, Boost Mobile, Cellcom, Cellular South, Centennial Wireless, Cincinnati Bell, GCI, Immix Wireless, Inland Cellular, IV Cellular, Nex-Tech Wireless, nTelos, Sprint PCS, T-Mobile, U.S. Cellular, United Wireless, Verizon Wireless, Virgin Mobile USA, and West Central Wireless.

Cost
There are no premium charges for joining UR Medicine MyChart Alerts, or for receiving the alert. However Msg & Data rates may apply.

Message Frequency
If you opt-in to receive alerts from UR Medicine, you will receive a text message when new information is viewable in your MyChart account (new test result, appointment reminders, message from your doctor, etc.).

How to opt-out of our service
To opt-out of UR Medicine MyChart Alerts, text STOP at any time to 1 (202) 990-8063. An unsubscribe message will be sent to your mobile number confirming the cancellation, but no more messages will be sent from this program after that one.

USE OF THIRD-PARTY SERVICES
You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor Hospital's MyChart have control over the contents of these services, and neither Epic nor Hospital's MyChart accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information and may be used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services. Hospital reserves the right to disable any 3rd party application if needed.

RESEARCH
Within MyChart you may receive notifications or messages from researchers about studies you may be eligible for. To learn more, log in and go to your Research Studies page. If you do not want to receive these messages, you must log in and indicate that you do not want to be contacted about research studies through MyChart. If you do want to be contacted, you can create a profile to select areas of research you are interested in, see what new opportunities you may be eligible for and what studies you are enrolled in. If you do not make a selection, we will assume that you are willing to be contacted.

PRIVACY POLICIES
Privacy Of Your Healthcare Information

The privacy of MyChart users' healthcare information is protected by state and federal laws including the Health Insurance Portability and Accountability Act (HIPAA). How we protect your messaging and medical information on MyChart
Any member of our health care team is able to access your MyChart messages to your health care team and information about your medical care. To protect your privacy, URMC employees receive privacy training and must adhere to HIPAA and other security requirements that include accessing the minimum amount of patient information necessary for that patient’s care. URMC privacy officers regularly audit such activity to ensure compliance.
